Optomap Retinal Exam

The Optomap is a comprehensive method of evaluating and monitoring various eye conditions. This previously unavailable medical technology assists our doctor in viewing the retina to detect early signs of eye disease as well as diabetes, heart disease, hypertension and cancer. This allows our doctor to determine the best course of action. We charge an additional $45 to perform this scan. You can visit Optos at: http://www.optos.com/us/.

In addition, retinal screening is being offered through Vision Service Plan for members as a "value-added" feature to complement their Well Vision Exam benefit.  These members will be eligible every 12 months.  The retinal screening charge will be $36 for each routine retinal screening.  This represents a discount of 20%.

For some VSP patients, the retinal screening may be a "covered" benefit.  Please ask us to check for coverage and/or applicable copayments.

Glasses

Patients without insurance:
(1) $99 for clear CR39 (plastic) single vision lenses* and frame (special collection). Additional $40 for polycarbonate lenses.
(2) $139 for clear CR39 (plastic) FT-28 bifocal lenses
* and frame (special collection).
(3) $249 for basic clear CR39 (plastic) progressive (no-line) lenses* and frame (special collection).
(4) All patients receive 20% off additional pairs of glasses. Must include frame and lenses.

CooperVision Rebate

The following rebates are for new wearers of Coopervision Avaira, Biofinity and Proclear 1-Day contact lenses. Regarding Avaira contacts, patients can receive rebates of $35 and $15 for 8 boxes and 4 boxes, repectively. Purchasing Biofinity contacts, patients can receive a $30 rebate for 4 boxes. For Proclear 1 Day (90 Pk), patients can receive $50 and $20 for 8 boxes and 4 boxes, respectively.
